I, the applicant, hereby give my consent for my child to participate in the Able Camps program in Thailand. I understand and acknowledge the nature of the educational activities involved, and I am aware that certain risks may be associated with these activities in 2026/2027.

1. Permission for Activities

I grant permission for my child to participate in all camp activities, including but not limited to outdoor adventures, water activities, and location-based educational projects. I understand that these activities may involve some level of physical exertion and potential exposure to the elements, and that travel between locations is handled by an independent provider as outlined below.

2. Transportation and Third-Party Logistics

​

I acknowledge that Able to Regenerate Co. LTD. operates strictly as an educational event organizer. All transportation to, from, and between educational locations is provided by an independent, third-party transport operator. I understand that the specific vehicle and driver will be assigned on the first day of camp based on participant numbers and age groups. Any transport fees collected by the camp are an administrative convenience passed entirely to the operator. I hereby release Able to Regenerate Co. LTD., its educators, and staff from any liability for injuries, damages, or losses occurring while entering, exiting, or riding in these assigned vehicles.

3. Health Information & Medical Financial Responsibility

​

I certify that my child is physically and mentally fit to participate in the camp activities. In case of any pre-existing medical conditions or allergies, I have provided complete and accurate information on the medical form. I authorize the camp staff to administer basic first aid and seek medical attention in case of an emergency. I confirm that my child is currently covered by a valid personal health or accident insurance policy that covers them for the duration of the camp.

I acknowledge that primary financial responsibility for any medical treatment rests with me. While the camp may arrange basic event-day accident coverage for participants, I explicitly agree that such coverage is subject to strict policy limits, and I am solely responsible for any medical expenses, hospital bills, or excess costs that exceed these limits.

4. PDPA & Third-Party Data Processing Consent
 

In compliance with Thailand's Personal Data Protection Act B.E. 2562 (PDPA), I hereby grant explicit consent for Able to Regenerate Co. LTD. to collect, store, and process my child's personal identification data (including Full Name, Date of Birth, and Passport/National ID number).

I authorize Able Camps to share this specific data with third-party contractors, partners, transport operators, activity providers, and venues (such as museums, farms, or ticketing authorities) strictly for operational logistics, age-verification, booking requirements, and health or safety compliance. I understand this data will be stored securely and deleted when no longer legally required for corporate compliance.

5. Emergency Contact Information

​

I have provided up-to-date emergency contact information, including phone numbers and email addresses, for myself and an alternate emergency contact person. I understand that every effort will be made to contact me or the designated emergency contact in case of any emergency involving my child.

 

6. Media Release

​

I give consent for my child's photograph or video footage to be used for promotional or informational purposes related to Able Camps. This may include, but is not limited to, camp brochures, the camp website, and social media. (If you wish to opt-out, please contact the camp administration directly prior to the start date).

7. Behavioral Expectations

​

I understand that my child is expected to follow the rules and guidelines set by the camp staff. In the event of persistent behavioral issues that may pose a risk to the safety or well-being of my child or others, I acknowledge that the camp reserves the right to take appropriate disciplinary action, including dismissal from the camp program.

8. General Release and Waiver

​

I, the applicant, release Able to Regenerate Co. LTD., its organizers, staff, and volunteers from any liability for injuries, damages, or losses that may occur during the educational camp activities, except those caused by gross negligence or willful misconduct.

9. Acknowledgment & Governing Law

​

I agree that this waiver and release shall be governed by and construed in accordance with the laws of Thailand.

Digital Consent: By submitting the registration form and checking the Release and Consent Form box, I confirm that I am the legal parent or guardian of the registered participant. I have read, understood, and agreed to all terms outlined in the Able Camps Participation Release Form, explicitly including the third-party transport waiver, medical financial limits, and PDPA data processing consent.
